How do I Begin Residency into Long Term Care?

Admissions Where do I Begin?

Step 1

First contact your local CCAC (Community Care Access Centre) where you will be assigned a CCAC Case Manager who will conduct an assessment of the individual seeking Long Term Care. You cannot apply directly to the Long-Term Care Home. Your Case Manager can assist you with:

  • Information about Long-Term Care Homes
  • Information about financial options, if required
  • Conducting an assessment in your home or in the hospital and determine your eligibility.
  • Help you complete the application forms and also coordinate the application process to facilitate access to Long-Term Care
  • Provide information on an alternative to a Long-Term Care Home, such as supportive/assistive housing, retirement homes and senior’s apartments
  • Place you on the waiting list, if necessary, of the Long-Term Care Homes that you have selected
  • Offer you a bed in your chosen Long-Term Care Home, based on availability

Step 2

Upon receipt of your assessment, the home will send you a letter stating whether  your application has been accepted or denied.

Step 3

Please contact the home to arrange a tour of the facility and meet with an individual who will be able to answer  specific questions about the home that you may have.

Step 4

If you are put on the waiting list you must wait for your CCAC (Community Care Access Centre ) Case Manager to contact you with a bed offer. The CCAC (Community Care Access Centre ) tracks and maintains the official waiting list. For details or if you have questions about the list, please contact your CCAC (Community Care Access Centre ) directly.

Step 5

When a room vacancy arises at your chosen Long Term Care home you will receive a bed offer from your CCAC (Community Care Access Centre ) Case Manager. It is up to you to arrange a date of placement/ admission with your CCAC (Community Care Access Centre ) Case Manager. You will be given 24 hours to make a decision.  Your Case Manager will then communicate the arrangements to the Long Term Care home.

Step 6

On the date of your entrance/ admission, a Registered Staff member will meet with you when you arrive at the home to assist you and welcome you to your new home.

Step 7

You must make arrangements as soon as possible (Day #1 preferred) to fill out the necessary paperwork with an administrative staff member at the home.
